What do I do if too much filler was injected? What if it looks blue?
Dr. John Burroughs is an oculofacial plastic surgeon practicing in Colorado Springs, CO. A considerable portion of his practice is managing complications or unsatisfactory outcomes from other practices. This patient had received dermal filler to improve her tear troughs.
Too much was injected and some had migrated inferiorly causing her lower eyelids and upper cheeks to appear puffy with a bluish-tint. Fortunately the filler injected was a hyaluronic acid, which can be injected with hyaluronidase, an enzyme. She received a single injection of the enzyme and it dissolved the filler. Some patients don’t want all of it dissolved, and sometimes the filler can be sculpted/contoured with the enzyme by varying the amounts, location, and concentration injected to improve the appearance. The patient is slightly smiling in the bottom (post-injection of the enzyme) picture, but she was very pleased to have her “Blue Bags” gone. Even with the lack of a flash in the bottom picture the blue appearance is completely resolved. Rarely, the enzyme is needed for emergency injections if dermal filler is cutting off blood flow to the site of injection or if it gets inadvertently injected into the nearby blood vessels as the dermal filler can cause a stroke in the eye that could lose to vision loss and possibly even blindness. There is a newer hyaluronic filler (Belotero) that won’t cause the blue appearance, but too much can still be a problem with lower eyelid injections.
